Part 1 Information about the person making the nomination

All nominations must be made by someone other than the volunteer nominee - this can be as an individual or a representative of an organisation.

Part 2 Information about the person being nominated

To be eligible for nomination for an award:

  • Nominees must be Australian Citizens or Permanent Residents with residency in NSW.
  • Nominees must be over 15 years of age
  • Nominees must have been involved in volunteering for the equivalent of at least 100 hours over a 12 month period.
  • Nominee must have volunteered for a legitimate not-for-profit organisation.
  • Communities in NSW must be significant beneficiaries of the nominee’s voluntary work

Describe the nominee’s volunteering work and the reasons you feel this person deserves to be recognised for the Award.

In preparing this statement, nominators should make reference to the following: an explanation of what is outstanding about the nominee and the work; a description of the benefit to the community; evidence of initiative and innovation; any obstacles overcome by the volunteer.

If the nominee has volunteered for more than one organisation, please provide details in the box below or send us details in the mail.

(Supporting evidence can be posted to "2008 NSW Volunteer of the Year Award c/o The Centre for Volunteering, 2/228 Pitt St, Sydney 2000", in addition to your online nomination. Please make sure to include the nominator and nominee's names and phone numbers with the supporting documentation).
 

Part 3 - References

All applications should include the names and contact details of at least 2 referees who can be contacted if necessary.

Nominators must secure the permission of the person being nominated
